Cardiothoracic Patient Navigator

The Cardiothoracic Patient Navigator for heart surgery patients at Halifax Health – Center for Cardiology works in collaboration with the UF Health Heart & Vascular program.

The Patient Navigator is an integral part of the treatment team. They work jointly with the medical team to provide education and information about our patient centered heart program.  They also help guide you through your treatment and assist in organizing your care to ensure you get the support and help you need.

As a patient, your first introduction to the Patient Navigator occurs after your evaluation by the surgical team or during your admission to the hospital. The Patient Navigator will review your diagnosis, treatment plan, and provide a smooth transition of your care from diagnosis through recovery.

They will also review your discharge plan with your and your family prior to discharge. Once home, you will receive a follow up phone call from the Patient Navigator to review important aspects of your recovery phase and answer any questions regarding the discharge plan.

The Patient Navigator is available to you for any questions or concerns throughout the entire recovery phase.
