Halifax Health’s Cardiothoracic Patient Navigator
Zena Neff, RN, BSN, CCRN helps to ease the burden through heart surgery.
The Cardiothoracic Navigator for heart surgery patients at is an integral part of your treatment team. The Navigator will work collaboratively with your medical team to provide education and information about our patient centered heart program. She will help guide you through your treatment and assist in organizing your care to ensure you get the support and help you need.
Your first introduction to your Navigator occurs after your evaluation by the surgical team or during your admission to the hospital. She will review your diagnosis, treatment plan, and provide a smooth transition of your care from diagnosis through recovery.
She will review your discharge plan with you and your family or caregiver prior to discharge. You will receive a follow up phone call from her to go over important aspects of your recovery phase and answer any questions regarding your discharge plan. Your Navigator is available to you for any questions or concerns throughout your recovery phase.
