While many hospice patients choose to receive care in their own homes, others find comfort at our Halifax Health – Hospice Care Centers in Port Orange, Orange City, Edgewater and Ormond Beach.
In all four locations, our patients receive loving, personalized care from an expert team of physicians, registered nurses, licensed clinical social workers, spiritual care chaplains, certified nursing assistants and volunteers. Every Halifax Health – Hospice Care Center provides management of acute symptoms hospice patients may experience during their end-of-life journey. Highly skilled interdisciplinary teams are available to provide inpatient levels of care 24-hours a day, seven days a week.
Above all, our Care Centers provide a serene and comfortable place where privacy and human dignity are honored while a patient’s final days can be lived out in the warm embrace of family and friends. Private patient suites provide a home-like atmosphere and common areas offer additional space for friends and family to gather. Loved ones, including pets, are welcome to stay overnight and each patient suite provides a pull-out sofa or sleeping chair to accommodate visitors.
Ormond Beach Care Center
Built to service our community in northern Volusia and Flagler County, this care center which opened in 2015 features 12 private inpatient suites. At first glance, you will notice that the facility features artifacts and many architectural elements to the historic Hotel Ormond. This center has a memorial walkway around the building with a gazebo for patients and families to enjoy.

Port Orange Care Center
This 16-bed care center is located on Dunlawton Avenue in Volusia County. This center has a memorial walkway around the building with a gazebo for patients and families to enjoy.
Each patient room features its own patio, bathroom, individualized climate control, private telephone, internet access and television. There is a great room for families to sit in with a piano and children’s corner. There is a Chapel available for patients and family use.

West Volusia Care Center
This 18-bed care center is located on five acres of wooded land and open fields in the heart of West Volusia. The architecture combines home-like familiarity with state-of-the-art efficiencies.
Each patient room is located in a six room pod with individual bathrooms, climate controls, telephone, internet access and a television. Each room has a sleep sofa to accommodate family members. In addition to the patient rooms, each wing of the care center has a sitting room for visitors. There is a Chapel available for patient and family use. This center has a memorial walkway around the building with a gazebo for patients and families to enjoy.

Southeast Volusia Care Center
This 12-bed care center offers a private, home-like environment with specialized care just steps away. It has a nautical theme which creates a casual atmosphere that is relaxing and soothing. There is a cozy family room with a fireplace and rocking chairs for all to enjoy. This center has a memorial walkway around the building with a gazebo for patients and families to enjoy.
Each patient suite has a private bath, scenic patio, individual climate control, private phone, HD television and internet access. Sitting rooms are available that include a sleep sofa that can accommodate two family members. There is a Chapel available for patient and family use.
