Lung Patient Navigator

At Halifax Health, we offer our patients a Lung Patient Navigator through Halifax Health – Center for Oncology. The Navigator is a registered nurse and is an integral part of your treatment team. The Navigator will help guide you through your treatment, assist in organizing your care, and provide education and assistance each step of the way. We want to make sure that you get the support and help you need.

The Lung Patient Navigator will empower you by supplying you with current information and education regarding your diagnosis, cancer treatment, management of any side effects of treatment and available resources. You will receive the appropriate monitoring and self-care education to ensure the highest quality of life possible during your journey.

Once you have been diagnosed and a referral has been made, Navigation services may include:

  • Assisting and arranging your care, taking you through the treatment options available
  • Performing a needs evaluation and referrals to appropriate resources
  • Primary contact during your treatment journey

Services are available whether you are inpatient at Halifax Health or being seen at the Halifax Health – Center for Oncology outpatient clinics.

Remember, you are not alone on this journey. We are all here for you and stand ready to support you in any way possible.
