Since 2008, Halifax Health – Center for Transplant Services has successfully performed more than 100 kidney transplants for patients with end-stage kidney failure. To elevate our program and continue our mission to provide exceptional care to our patients, Halifax Health entered an affiliation with UF Health Shands Transplant Center in 2017. UF Health was the first transplant center in the state of Florida to complete a kidney transplant. This affiliation gives us the ability to help more patients and save more lives.

The Mission of Halifax Health – Center for Transplant Services is to provide professionally integrated, expert medical and surgical treatment and follow-up care for our patients. In accordance with the vision and mission of Halifax Health Medical Center, the Center for Transplant Services will meet the needs of transplant candidates, living donors, transplant recipients and their families by ensuring excellence, accessibility, and compassion.
Kidney Transplant Evaluation
For individuals who are considering a referral for kidney transplant evaluation and meet the following criteria for candidacy: Stage 4 Chronic Kidney Disease (GFR < OR = 20) or stage 5/End Stage Renal Disease (on dialysis).
