Proxy access allows a trusted individual to view a patient’s health information through Halifax Health’s MyChart Patient Portal.
Proxies can be parents, legal guardians, healthcare surrogates, or individuals with power of attorney; however you can give anyone you trust proxy access.
Improved Care Coordination: Proxies can view test results, medications, and upcoming appointments, helping patients stay on track with their care.
Convenient Communication: Proxies with full access can send secure messages to providers and assist with scheduling visits.
Support for Vulnerable Patients: Proxy access ensures that minors, elderly patients, or those with disabilities have someone to help manage their health.
Secure and Controlled Access: Halifax Health uses robust identity verification and legal documentation review to ensure only authorized individuals gain access.
Full Access: View all health information and use all portal features (messaging, scheduling, etc.). Enables full use of portal features: messaging, scheduling, eCheck-in, and more.
Clinical Access: View-only access to health information; no messaging or scheduling. Ideal for individuals who need to monitor care but not manage it.
Parent Accessing Child: Full access to a minor’s health information (excluding sensitive data). Enables full use of portal features: messaging, scheduling, eCheck-in, and more.
Power of Attorney: Full access including sensitive information and ability to act on the patient’s behalf. For individuals legally authorized to act on behalf of a patient. Requires submission of a valid, properly executed POA document.
For Adult Patients
Self-Initiated: Patients can send proxy invitations directly through MyChart.
In-Person: During a visit, both patient and proxy can complete the process with staff assistance.
By Phone: Patients may request proxy setup via a phone call to Halifax Health, with identity verification.
For Minor Patients
New Mothers: Proxy access will be automatically granted to mothers of newborn’s delivered at Halifax Health as of January 31, 2026.
Everyone Else: Everyone else must complete the Proxy Authorization Form and present valid photo ID. If parental consent is unavailable, supporting legal documentation is required for those wishing to establish access as a Power of Attorney, Healthcare Surrogate or similar relationships.
