Halifax Health - Care at Home Achieves Five-Star Quality of Patient Care Rating

Port Orange, FL – (August 13, 2024) Halifax Health - Care at Home has received a five-star rating in the Quality of Patient Care Star Rating from the Centers for Medicare & Medicaid Services (CMS). This rating is based on end-of-care OASIS assessments and Medicare fee-for-service claims data for the period from January 1, 2023, to December 31, 2023.This rating reflects how home health agencies (HHAs) compare with one another on their quality of patient care performance. A five-star rating signifies that Halifax Health - Care at Home has performed exceptionally well, surpassing national standards and delivering exceptional care.

The Quality of Patient Care Star Ratings are determined using seven measures of quality reported on the Care Compare website. These measures include:

  1. Timely Initiation of Care

  2. Improvement in Management of Oral Medications

  3. Improvement in Ambulation

  4. Improvement in Bed Transferring

  5. Improvement in Bathing

  6. Improvement in Dyspnea

  7. Home Health Within-Stay Potentially Preventable Hospitalization (PPH)

Effective May 2024, CMS replaced the Acute Care Hospitalization measure with the Home Health Within-Stay Potentially Preventable Hospitalization measure, emphasizing the importance of preventing hospitalizations during home healthcare.
