Your First Visit

At Halifax Health – Primary Care, our goal is to ensure that your first visit goes as smoothly as possible.

On your first visit, we will begin a permanent record. Please bring any medications you are taking, so that we may accurately determine the dosage and frequency. We will ask you to complete a medical history and registration form. Please bring your insurance card and arrive at least thirty minutes early. If you have been under the care of another physician, we may ask your permission to transfer records.

We have three appointment categories:

  • Annual Exams or New Patient Visits – This visit should take approximately one hour and will consist of check-in, form completion and physician visit

  • Follow-Up Exams – This visit should take approximately thirty minutes

  • Sick Visits – The duration of this visit depends on the illness

If you find that you are unable to keep your appointment or are running late, please call your physician’s office as soon as possible. This courtesy allows us to assist other patients.

If you have an urgent problem, you may call your physician’s office to schedule an appointment. However, if you feel it is a life-threatening emergency, please call 911.

At Halifax Health – Primary Care we have a strict patient confidentiality policy. Your medical record is strictly private and will not be shared with your employer, friends or relatives without your written permission.
